Wellness Systems Recognized by IHI

Age-Friendly Health Systems is an initiative of The John A. Hartford Foundation and the Found for Healthcare Improvement (IHI) in partnership with the American Hospital Association (AHA) and the Catholic Health Clan of the United states of america (CHA). As of August 2021, there are over 2500 recognized Historic period-Friendly Wellness Organization participants.
Five pioneer health systems stepped forward to piece of work with IHI to test ideas and learn what information technology takes to be an Historic period-Friendly Health System. The 4Ms Framework for Age-Friendly Care that emerged is both evidence-based and able to be put into do reliably in health care settings.
